如您有呼吸系統疾病（如氣喘、氣胸等）的病史,曾有精神或神經系統疾病病史（如癲癇、恐慌發作等）正處於術後追蹤觀察期間，或患有糖尿病、潰瘍等慢性病年滿45歲且具有高血壓或吸菸史等風險因素，請事先諮詢您的醫師。
若需要醫生評估檢查表，請自行列印此PDF檔案交由醫師簽署。
前來沖繩參加活動時須提供正本文件以資證明。
未能提供相關文件者，基於安全考量，將無法參加活動。
年齡限制：
浮潛可參加年齡：5～65歲
水肺潛水可參加年齡：10～65歲
60至65歲的參加者需要提供醫生開立之醫生評估檢查表。
